i dont know how old your bub is but other reasons (out of the normal like tired, hungry, bum change) could be colic or reflux...   

yeah shushing has to be loud so they can hear it over their screaming!

Hang in there... he will stop eventually and sometimes you just have to go thru the check list (bum change, feed, wind, swaddle and put down) over and over until something clicks...

Big hugs Floss! Twins are hard work! I second what everyone else has suggested, but more importantly, do whatever it takes to make the crying stop. I spent too much time worrying about setting up bad habits with the gremlins and wouldn't rock/pat them to sleep and sometimes it felt like they screamed non stop for days, I wish I had just bloody well rocked them so we could all have got some sleep!

ETA Another you can do in the pram and rock it backwards and forwards over a bump like were the lino meets the carpet.

Spencer use to get a bit colicky and cry in the evenings at that age I found laying him face down across my knees and rocking him back and forward whilst rubbing/patting his back would normally calm him down.
He also liked being walked up and down, sitting and rocking wouldn't do.
Over the shoulder can work quite well too, just remember to put a cloth on the shoulder first.

Edited to add have you tried giving him a bit of gripe water....colic normally occurs in the evening and the symptom is crying for more than 2 hours for no apparent reason.

Floss, Tom went through a phase like that and thankfully it was only short lived ( a week or 2). I remember being in tears as I didnt know what my baby was trying to tell me. Then I read that babies can cry for about 4-6 hours a day FOR NO REASON (Great!!!).

Dont worry about doing "the right thing" Do what you feel right about. If you want to pick him up and cuddle him then go for it. Walk around with him. Tom didnt like me sitting down rocking him. Shhhhhh really loudly. Have you tried singing???? Tom quietens down immediately when I sing edelweiss from sound of music (dont ask how we know that). It is freaky.

ah, thats right twins...

i remember once with gabriel he wouldnt stop screaming and it turned out his dad had got his balls caught under the nappy when he had changed his bum. Oh man, poor You and poor Noah!
Other than the 4 usual suspects (hungry, dirty bum, tired or wind) you could try the 5 s's.
That worked an absolute treat with Ella who was a real screamer.
You lay bubs on their tummy across your forearm cupping their head in your hand, gently sway them from side to side in a rhythmic motion while making shhhhhh noises. I forget what the other s's are for but those 3 definitely worked for us and you can google it for the full rundown.
I found it great for calming her down so she was ready to sleep or feed.
Go really hard with the shhhh'ing too, they don't mind it loud as it reminds them of womb noises. alternatively you can try un-tuning the radio to static or running the vacuum cleaner.
White noise is awesome for calming them.
Good luck hun, I hope you find something that works soon.
