Jess, my daughter Caitlin had really chubby thighs and a slim-ish waist as a baby. I found the only thing that would accommodate both was a nappy with pointed wings (like Fuzzis and Honeychild) as you can do them up to fit the thighs on one setting and use a completely different setting around the waist without it effecting the thigh settings.

I'm afraid I don't have any experience with prefolds & covers but my son had chubby thighs and we got on really well with both Fuzzi Bunz & Honeychild pocket nappies.
