Ergo here too and I just downsize what I take with me. Backpack here, though I invested in a nanna trolley for when we walk to the library or markets and I know I'm going to be hauling a heavy load.

I agree that is one down side to baby wearing. And I'm sorry but I refuse to wear a backpack!
I will carry my big nappy bag on shoulder. Its a bit awkward, but oh well. Also, once baby is older, I find I don't need to take as much stuff. Usually I just leave the bag in the van while I'm shopping. Manduca's do a pouch which can attach to the carrier or on a strap..

I usually take my over the shoulder bag. Or else if I'm not going out alone my buddy takes a nappy/wipes/my wallet

Sometimes I use my nappy shoulder bag but now have started using a small backpack (baby on the front), which is heaps easier.

If its a short trip I'll stuff what I can in my handbag, otherwise I'll pop DS on my front and backpack on my back. This may seem like a stupid/weird question but for those that use slings during the day what do you do with your nappy bag when go out?
DS generally doesnt like pram but does like being in the front pack so I use that often but I dont use if I think I will need something (like clean nappy or burp cloth) from the nappy bag as I dont know what to do with the bag as I imagine it would be awkward with DS in a front pack or sling and carrying a nappy bag.
