Mya loves everything cold, even the formula, so hardly bother heating things up. I use it to heat bottles and baby food too. I use it if I'm in a real hurry for a bottle. Usually just heat the 200ml for 20 secs (800 watts), then add the formula. The formula does mix easier, and for such a short burst of heat, the bottle can be well shaken and used straight away - never had a problem.

The only reasoning I am aware of is as you have already mentioned, re: hot spots. I just make extra sure the bottle is evenly shaken, and usually leave it to stand for a little while too. No idea. Maya's daycare used a microwave.
