Thanks ladies. Had my second beta today, and all is progressing as it should. Hopefully get to stop the pessaries soon! Scan hasn't been scheduled yet (I can't remember from last time, but I think it must be after 3rd beta that they schedule it), but should be in 2-3 weeks.

Sorry Miswe we’ve never had sperm count issues so I can’t offer any advice but I can understand your frustration. There’s nothing worse than feeling you’re stuck on the hamster wheel getting nowhere.

That’s very cool Roubaix!FX the first tri treats you somewhat nicely

My appointment got cancelled 😑 again. So I’ve been rescheduled to the 11th. So doing nothing this cycle which is extra frustrating as expected to be doing IUI at least. But a forced month off is probably not a bad thing for me to have to do!

That's really frustrating for you Miswe. I've always hated any delays in treatment, even though like you say they're probably a good thing for your body!

Hi all,

We are new to the infertility journey and are after some advice. We are aged 32 and 33 and have been trying to conceive for 12 months. My cycle has become increasingly irregular and I have had some anovulatory cycles.

We have already had quite a few tests done through our GP including full preconception bloods twice, progesterone tracking for ovulation, an ultrasound (came back normal) because I was having a lot of pain at ovulation and had had a period that lasted 15 days (possibly an early miscarriage), sperm analysis (came back normal), we are also about to do an AMH test.

We have just been referred by our GP to a fertility specialist via the public system but have not heard anything yet. We have read that the wait for an initial consult can be up to 3 months and we are not sure we want to wait that long to even find out if we will qualify for public funding.
We have been to an information evening at Genea Oxford in Christchurch and they seem great. Has anyone used them and would you recommend them?

So my questions are:
How much will an initial consult cost?
Will we save money because we have already had a lot of tests?
What are the pros/cons of going private?
If we go private and then qualify for public how does it work for transferring?

Hi Gina

I can't help you on Genea specifically, but in general the initial consult will be somewhere in the range of $200-$300. I would recommend paying the money for the consult because they can score you for public treatment and put you on the waitlist if you qualify. I don't know what the current wait time in Chch is once you're on the list, but average throughout NZ is about 12-18mths. If you don't qualify for public treatment (eg. if you're unexplained you have to have been trying for 5 years to get on the list), then you can make the decision on whether to jump straight in to private treatment. Either way, you're 3 months ahead, and have a better idea of what treatment you're likely to need.

You should save a bit of money because you've had initial tests, but if you don't have a copy of the results from those tests, try and get them so doc can see exact results. Otherwise they may want to repeat them (esp. sperm analysis).

Pros and cons of private treatment - the two main things are time (pro - no waiting other than for your body) and cost (major con - you should allow around $15k for a full round of IVF - but don't freak out about that as you might not need IVF). As for the time thing....I'm currently 6 weeks pregnant with my second child, and only got the call that I was at the top of the waitlist a month ago (I'd already started this private cycle). We first started trying when I was 32 and I'm now 39. (Admittedly I would have got the call a year earlier if I hadn't had my daughter - the year not trying to conceive whilst I had her meant it was 6 years since we first started trying rather than 5 before I got on the list). But if I'd had to wait for public treatment, even if it had worked first time, I'd only just have had a baby in the past couple of months, instead of being pregnant with my second.

If you go private, once you reach the top of the public waitlist you have to have finished any private cycles (eg. used all frozen embryos), before you can start public treatment. If that's the case, you don't get dropped to the bottom of the waitlist again, they'll just defer your treatment a month (or however long) until you've completed the private cycle.

Hi Gina

Sorry you're in this situation! My GP referred me to a specialist which it turns out (after a 4 month wait) was just an OB at the hospital. He was useless and offered to refer me to Fertility Associates (our local) but I decided that we didn't have time to waste another 3 months so we went privately for our first appointment. I believe that cost $250 a few years back.

From our appointment and test results we were put on the public list for a funded round of IVF. That took 18 months to get to the top of. In the meantime we paid for some rounds privately. Unfortunately while none of those were successful they did allow us to diagnose some other problems. If you do need IVF it's important to note that it's largely diagnostic. I went through various protocols before we found the one which worked. Some people have success early on while others it takes rounds and rounds. We had success on our fourth round which was the public funded round.

Your clinic may want to re-run some tests. My husband had to pay for another sperm analysis but all my tests were covered. I never had the test to see if my tubes are open (that would have been covered if I continued with the OB at the hospital) but it turns out that we needed IVF no matter what so it doesn't really matter if they are open or not.

There's a lot of waiting when it comes to fertility treatments so I'd recommend taking the first leap into a private consultation if you can afford it. They might be able to do something to help you ovulate while you wait on the public list such as femera or clomid.

Hi Gina,

Roubaix and AnyDayNow have covered more than I know, but in terms of fees there's a breakdown of costs for all treatments on the fertility associates website. Not Genea, but I imagine that they're all in the same ballpark. Cost of meds vary depending on what protocol you're on. We also got told that we could be referred for a public consult, with a wait of around 3 months. But we were sure that we weren't going to qualify for public funding anyway so decided to just go private. If you go private then you can pretty well start whatever you're going to be doing straight away.

AFM had our appointment today, couldn't really find any reason for the difference in sperm count, going to run some blood tests but he was pretty doubtful that there would be a cause in there. Ordered and USS in case there's a sneaky varicose vein or something blocking there. However, our best shot is still IVF. Even if they find a vein, he'll have to get it fixed ($$$) and then it's a 4-5 month wait to see *if* his count goes back up.

Have got the script to start progynova at the end of the week if we want to go ahead with next cycle. Now just trying to wrap my head around it all. Certainly never thought that we would be going down the IVF road! It'd also be ICSI, so super lab heavy :/

Great that you finally got your appointment, Miswe. I'm sorry that it's looking like IVF. I recommend ICSI as I've read horror stories of people who have ended up with no fertilised eggs from IVF without ICSI. It's an extra step but not really more lab heavy. Good luck for the round!

Thanks ADN! I just meant more medically involved... not really the right words... like at least in IVF they’re still swimming somewhere 😂 but good to know about the horror stories!

How did everyone do with wrapping their heads around the money involved. I suppose it’s just like anything. But it sucks we have to spend so much to do something so many can do for free.

Miswe - sorry there's no real clear answer for you. But ICSI is definitely a good thing (except for the added cost) - my first cycle was just straight IVF as theoretically there's nothing wrong with DH's swimmers, and of 7 eggs collected only 2 fertilised. The next 3 cycles were all with ICSI and fertilisation rates were SOOO much better - 5 of 8 eggs, 5 of 7, and 3 of 4.

The money is hard. We've been lucky enough to have 4 rounds privately only because family have been able to help us. And if this round hadn't worked we wouldn't have paid for any more (and I was having doubts about whether or not I'd put myself through a 5th round even though it would have been public). BUT my little girl is so worth everything we've been through - physically, emotionally and financially - and I've willingly done it all again to try and give her a sibling.

I agree, the money is definitely hard. We've had to make sacrifices for IVF for sure. We paid for two full and one cancelled (poor response) round of IVF out of pocket. It's really demoralising when sometimes you've put all this money into failure or miscarriage. It's ok to be pissed by that.

Interesting to know those stats on ICSI Roubaix! Funnily enough back in the day I would’ve spent hours researching this. Now I just roll with it.

I think that’s the thing, if we’d had to do this to get my daughter I wouldn’t even think about it. Though I am worried that once we start I don’t know if I can draw a line and we know the 50% chance is a pretty random ballpark!

Hey team, it's been a while... 
Got a call from my clinic saying would I like my publicly funded IVF round next month... only about five months earlier than I was told to expect so I'm slightly in shock but excited too. Although I'm booked to go to Samoa next week and given that's a zika risk destination I'm wondering if that will delay us... we wait to hear.

Miswe, you'll be on the countdown. We will do ICSI in our next round and I'm really hoping it will help our fertilisation rates - we only got 1 of 6 fertilised last time which was gutting (and even more so because we were paying privately for that cycle). Great to hear others have found it helpful. I get what you mean about more intervention but then again if you're successful I don't think you'll ever regret it. 

Gina, have you managed to see Genea yet? We went to them in the early days, after being mucked around through the hybrid public/private system that used to be OFS in Dunedin and they were great - really proactive and made us feel like we were making progress. If you've had irregular and anovulatory cycles they might try Letrazole or Clomiphene for a start which is not very expensive - a few hundred dollars a cycle to do the meds and have them monitor you for ovulation. We decided to privately fund everything except IVF when TTC#1 - we figured that a few hundred here and there was worth a punt to jump ahead of those waiting for public consultations. I also got a laparoscopy done to diagnose and remove endo through their sister clinic Oxford Women's Health which was covered under my health insurance - again something I was told I'd have to wait months for through the public system. Once we went to public wait list for IVF we got transferred to Fertility Associates (because they're now the public provider for Southern DHB), who have also been really good. It's so hard when you don't know where to turn and it feels like everyone is telling you to just wait... the group here is a great source of knowledge though I have found these ladies really helpful over the years so anything you want to know, just ask! 

How's your pregnancy going Roubaix? You're so right about the TTC journey fading into the background once you have your take home baby. Your wee girl is a similar age to mine... they bring so much cuteness and joy huh. Is she excited that she's going to get a sibling?

Wee, that's super exciting Star! Hopefully they will let you proceed even with your trip to Samoa. I hope you have a fab time there. We postponed after our Fiji trip due to Zika risk but I don't think there's that really a high risk in the Islands especially if you stick to the resorts and use all precautions. Harry & Meghan went to Fiji when Meghan was pregnant and I'm sure they had advice on that.
