I take both as i have everything in a particlular order in my hand bag and know where it all is without having to rummage and I make DH carry the NB anyway I don't like the NB being so full that I have to pull everything out to find something.

Really depends on where Im going. If im only popping to the shops or out for a coffee then I just take our smaller mountain buggy nb which has lots of pockets in the front for my cell and Wallet etc.
If I go out for more than a few hours then I take a bigger nappy bag plus my purse.

My nappy bag has a separate zip off handbag portion, so if I'm just whipping out to the supermarket or something I'll take only the handbag part, and if I'm heading out with JJ for longer I'll zip them back together and take it with the nappy bag as one unit.
The nappy bag has spare nappies, wetbag, wipes, hand sanitiser, change mat, change of clothes, moby wrap, hat, baby banz and baby sunscreen. The handbag portion has phone, camera, wallet, keys, lip balm, glasses, pen and paper. I haven't been carrying pads but thanks for the reminder, I probably should, Murphy's law says AF will return while I'm out and about!

I always take the nappy bag if Daniel is with me - nappies, change mat, wipes, bum cream, cloth nappy for spills, suntan lotion, sunhat, 3 toys, 2 changes of top, 2 bibs (he's very spilly), 2 dummies, his medication + syringes, sunglasses, my wallet, phone, pens, tampons (just in case), lip balm, hand cream. Mine has 3 pockets on the outside which I use for my stuff, plus 2 insulated pockets where I put his meds. Everything else goes on the inside which has 5 pockets for stroing stuff.

Nappy Bag only for me unless I'm going out without baby in which case I'll transfer phone, sunnies, wallet, lipgloss to a little handbag.
Nappy bag has:
Nappies, wipes, spray, cream, nappy sacks, (or cloth nappies and wetbag depending on what Rowan's using at the time)
container of snacks,
drink cup,
3 or so little toys,
change mat,
my sunhat,
Rowan's sunhat,
Rowan's sunnies,
baby sunblock,
warm hat,
socks,
jumper,
bib
and then I treat the external pocket as my 'handbag'
ETA: I think there may be tampons, pads and a pen somewhere in it as well.

I've had both on the go since starting back at work but before that it was all in the baby bag. In there are:
Two nappies
Wipes
Plastic bag for dirty nappies
Change of pants
Container of raisins or some other kiddie-friendly snack
Snack for me (because I am gluten intolerant)
Wallet, keys
Colouring in pens & paper for the older girls (depending on where we are going)
You guys seem to pack a lot more than me but don't worry, I'll be back at that stage soon
Things we carry in the van regularly but not in the baby bag are:
Sunblock
Dolls/blankies (if going somewhere boring or at a time that would usually mean a sleep for someone)
Drink bottles in a cooler bag for 5 of us
Good thing we have a van

Wow and I thought I had alot in my baby bag! I am trying to cut down with #2 on the way!
I carry both as I got sick of leaving wallet/phone etc behind if I was only nipping out or going by myself.
Handbag - a little MB one we got at the P&C show
wallet
phone
sunglasses
keys
lip gloss
I don't think there is any rubbish in there anymore as I cleaned it out a while ago
Nappy Bag
MCNs - trying to cut down to 2 unless we are out all day
wipes
nappy bags for dirty nappies
arnica spray and cream!
change of clothes (must check if they still fit!)
Depending on where we are going - suntan lotion and hat, food and drink
My handbag sits on top of the nappy bag if I am taking both
