Topic: What to expect at first FA appointment... Posted: 06 July 2010 at 11:37am |
Hi Ladies
Im just wondering what we can expect from our first FA appointment which is on the 2nd August.
A little bit of our TTC history... we got pg back in 2008 after trying for some months but unfortunately that ended in a mc at 12weeks. We have been AT since then and last month I was diagnosed with PCOS.
We have had all the blood tests, SA done & I’ve had a scan which found the cysts and Im wondering if they will try Clomid straight off or will they want to do more tests before?
Thanks in advance

Hey wantabub, I have just had mine. She wouldn't put me on clomid because i would run the risk of having twins because im young, so to speak...
I have to do day 21 and 25 bloods and also cycle tracking with the nurses at Otago Health. That involves more scans and bloods at each stage of my cycle!
At the appointment she had a feel around to see if she could feel anything abnormal...nothing found, so it is the cycle tracking for me, if they don't find anything she suggests we do a larascopy (spelling?) which will show things happening with ovaries etc, this is dont under a local.
But really i'm not sure as they have detected cysts with you, they might just put you straight onto clomid.
Sorry i can't be of more help.

Hi Wantabub,
This is what happened at my first FA appt, hope it helps.
When i rung up and booked an appt (post referral) they gave me a blood lab form to get some intial tests done and DH had to do a sperm sample to get checked(- much to his disgust!) At our first appt we went over the test results and discussed our history.... any prev children, cycle lengths- whether they are reg or irreg, how long we'd been trying, what contraception etc we had used over the years, family history etc.
During the appt I had an internal scan to check my ovaries , found I had PCOS and then discussed what that meant and treatment options.
I was prescribed clomid and got given a 3 month supply with 3 months worth of blood test forms for day 21 progesterone checks. I didnt have monthly scans, but some of the lovely ladies in the clomid thread did to see if eggs were forming....
Best advice is to have those bloods etc done pre-appt and if you have time (which you prob dont) have a small calender that tracks your cycle lengths etc so your specialist can see whats going on easily. I had mine colour coded with spotting and actual bleeding....
I wish you all the best, without FA I wouldnt be expecting my first baby girl, look forward to hearing how you get on, take care

Thanks Emma for the reply! Its good to know what kinda things they'll ask etc so I'll make sure I take all my charts with me. I wouldn't mind twins but Im a few years older than you. I hope it doesn't take too long for you to get some answers...
Mindy thanks so much for all that info... good to know that they hopefully will give me clomid at my first appointment. I hope the clomid will work for me as it seems Im not ov but last time I had my bloods done was in Feb when I did ov Do you mind me asking how long it took you to get pg with PCOS. All the best for the birth of your wee girl! How exciting...

I did 10 cycles of clomid- a very hard and frustrating time! I also had a lap surgery and got preg the month following. Without the clomid i wasnt ovulating at all, so I think it was a combination of the surgery and clomid that got us our BFP.
All the very best wantabub and look forward to hearing about your BFP

My first appointment was very similar to Mindy's. Since you've had most of the diagnostics done, I imagine that they'll talk about PCOS and work out a plan forward - clomid would seem like an obvious option.
As to how long it took to get pregnant with PCOS, I didn't really respond to clomid - after trying for a while (in which I oved twice) they classed me as clomid resistant. We got pregnant with this Bub after they did a lap where they cleared some endo (which I also had) and drilled my ovaries. Hopefully you respond better to clomid than I did!

They then went through all our options, costs etc and gave us a dvd to watch

I was wondering,Wantabub, how long did you have to wait for FA to get hold of you for your appointment? (I've also asked the q in CPP, but see you are in Wellington and you're appointment is soon)
I saw the hospital gyn last Monday, and he said he would be referring us to FA, so of course he has to write the referral, then they have to process it etc. Will they let me know an appointment date as soon as they get the referral or will they wait until it's just a couple of weeks out from the appointment date before letting me know? I'm sure they're used to people ringing up and asking, but I'm hoping to hold out for at least another two weeks so I don't seem too obsessive

Hey ArielAngel
I've got a few questions for you too (and anyone else who can help me) but first I'll answer yours!
I had a bit of a hold up cos my dr sent my referral to the gynae when I thought it was going to FA so it was a little slower than what it should have been. But when he did send the referral I think it was a Friday if I remember right and I think I called FA on the Tue and they had an appointment for me so no it doesn't take them long to set the appointment and then they send a letter and forms for you to fill out. Then my appointment was about 6weeks away so it is a bit of a wait
My question is, obviously you saw a gynae first. Can I ask, don't answer if you don't want but what did they do? I've got a gynae appointment next week and FA early Aug but I doubt I need both aye? But I don't know which appointment to keep... do you think it was worth seeing the gynae first? Sorry for all the questions and please only answer what you feel comfortable with

Thanks Wantabub. I might give FA a call later today then (as it has been over a week) - we've already filled in one form for FA as DH had a SA done there. We never got an invoice for the SA, so might follow up on that at the same time (and gives me a bit more of an excuse to call! )
In regards to your question, my story is quite long. After going off the pill early last year I had really irregular periods and so went to my doctor who ordered some blood tests, and when these came back a abnormal I paid for an ultrasound to be done. The ultrasound showed a mass so I was referred to hospital gyn and had further ultrasound - this showed mass had gone but that I had a heart shaped uterus. I had a whole bunch more blood tests which confirmed I wasn't ovulating and had high prolactin levels. I had over a six month wait for my MRI for my uterus but finally had minor heart shape confirmed but surgery not necessary (phew!). I had more bloods which showed my prolactin levels dropped so didn't need any further tests in this area and gyn finally could confirm PCOS (it isn't really obvious in my case (both cysts and bloods were inconclusive) so until they ruled out all the other reasons for not ov (like the prolactin), they couldn't confirm that diagnosis). Gyn is going to refer me to FA, who may refer me back to him if they run further tests and decide I need surgery.
So I guess, my answer would be, if you don't know of any other problems, then maybe the gyn appointment isn't really necessary. It could be though, that your dr referred you to the gyn due to something else on your scan - I'm not sure. I would recommend calling your dr or FA and asking.

Hmmm thats interesting ArielAngel... I talked with a friend who is on the IVF waiting list and she said to go with the gynae as they can do everything that FA can do apart from IVF and everything is way cheaper. She said her friend paid $100 for and IUI So after talking to her I thought I'd go to the gynae appointment on Thursday and see how that goes. If Im not happy I'll go to my FA appointment early Aug otherwise if it goes well I'll cancel the FA appointment I think. Did they give your gynae give you clomid or do you have to go to FA for that?

Hi Wantabub.
I went to my gynae first before FA, I also have PCOS. The gynae had me on clomid, but didn't monitor me with bloods, so the timing was all out. I wasted 6 or so months taking clomid and half the time I wasn't even ov.
I found FA very methodical and focused, when clomid wasn't working (as I was resistant to the drug) they added hormone injections to kickstart the ov. When the clomid didn't work, we swapped to OI injections Finally concieved after 4 months or so of them... Prob 3 years trying all up
If you have a good gynae, and then you might be okay without FA, but I have to say I found the service well worth the $$. My son is prob worth $20K at least. (I'm too scared to add up all the bills and see!!) Note in wtgn you pay for all blood tests from private specialist. It was about $30 a test, and I would have 10 or so per month, so that made up a large part of the cost.
BTW I reckon the whole thing only worked the on our last try because I put myself on a real strict low GI diet and didn't touch caffeine.

Thanks HuMum for your reply!
Thanks for sharing your experience with me... its really good to hear what people have been through going with a gynae or FA! Im going to go to my gyane appointment this week and see what they say and then decided if I'll keep my FA appointment or move it back a couple of months.
Did you see a gynae in Wellington? Im seeing one at Wellington hospital, Dr Murray Team.
Yeah I've been looking into a low GI diet so really need to get strict on it I think.

Sorry, haven't been on here for a few days.
My gyn would have prescribed me clomid but like HuMum said, it wouldn't have been monitored. Because we have multiple fertility issues I feel that FA is the best place for us and to be fair, I don't want to rely on the public health system - even when I booked a gyn appointment just after I'd had one, I still had to wait a month. But my gyn was good - Dr Peter Ables at Wellington hospital - he fought really hard to get me my MRI after I'd been waiting six months.
Good luck for your appointment this week - I would recommend making a list of questions and taking them with you, whenever I went there would always be at least one thing I would forget to ask!
