   Hi there
 
 Im currently doing Kylahs passport.
 Im going to go to a passport photo place in Christchurch to get the photo. Any recommendations? Also is it really hard? I cant imagine Kylah will make it easy lol.
 
 Also, i have to send her original birth cert, will i get that back?
 
 Also the witness bit.. I dont know anyone who meets their requirements.
 Will her old doctor (iv just changed) still have files to say she was her doc for the last year?

   Hi Kandice - if Kylah was born in NZ you DO NOT need to send in her birth cert. They have a great computer system that links the Citizenship, Passports and BDM (births, deaths & marriages)records.
 
With regards to a person for the identity section it does not need to be a person in "authority" just someone who holds a NZ passport, is not a relation and does not live with you  (oh, and is over 16)

   What about contacting your MW to be the witness?  Or you can use your Dr, or a close friend of yours.

     TTC 6 years IVF it is  IVF/ICSI round one 10 eggs, 8 mature, 3 fertilised BFN IVF/ICSI #2 = 22 eggs! 20 mature, 15 fertilised, 1 fresh transfer and 2 frosties BFN 2 Frosties still in freezer thank god

 Do you fit the following groups?
 
 Lawyer
 Police Officer
 JP
 Teacher
 Kaumatua
 Applicants Employer
 Minister of Religion 
 Resgistered medical professional.
 
 It then has two boxes
 
 Yes = Tick which applies and type your addrewss
 No = The person applying for passport must look for someone else who meets witness requirements.

   that is only if you can't find someone who meets the other criteria :
 Be aged 16 years or over; 
 Be the holder of a valid New Zealand passport; 
 Have known the person applying for the passport for more than 12 months (or since birth for a child under 12 months); 
 Have a daytime contact telephone number; 
 Be contactable during normal business hours; and 
 Be willing to provide the required personal details such as their date of birth and home address.
 
 What if I can't find a witness who holds a valid New Zealand Passport?
 If a witness cannot be found who holds a valid New Zealand passport, choose someone from the following groups to act as the witness. They must meet all the other witness criteria mentioned above. 
 Lawyer 
 Justice of the Peace 
 Sworn Police Officer 
 Applicant's Employer 
 Minister of Religion 
 Teacher 
 Registered Medical Practitioner

   I've just done mine for my younger kids.  Our doctor signed for both of them.  
 She didn't have to include her passport number because of the fact that she was a doctor... So yes you could get your family doctor to do it for you.
 
 Also in regards to the passport photo, Cooper was really obliging, he sat on the chair, and was so taken by the camera that he just there, they managed to get a really good shot... they'll keep taking one until they get one that is suitable (well our Camera House did).  Don't forget to get the stickers for the back of the photos too.

   OMG im blind. It clearly says in bold letters, if the person has a passport continue to Q 43, which bypasses the police, lawyer etc..
