Topic: Placenta previa, when did it move? Posted: 24 June 2010 at 10:11am |
I had an episode of bleeding last week and found out early this week that I have a complete placenta previa. I know that most move out of the way but at what stage in your pregnancy did they move? I'm very worried about going into preterm labour again and it wouldn't have had time to move enough, as from what I have read, most seem to move after 30 weeks? I am already under a high risk team and being seen and scanned monthly so I guess there isn't much else they can do

Mine wasn't complete with Daniel but there was less than 1cm before it hit my cervix so it was getting pretty close which I found out at 20w. I had a scan at 30w I think, maybe 32w and it had moved well up and away.
Its good your being monitored anyway

Oh that must be scary for you Kelly! I'm by no means an expert but I believe the earlier the PP is diagnosed the more likely it is for it to grow away from the cervix because the uterus has more growing to do. It depends on the grade too and I assume yours is a Grade 4 given that it is complete. Because I'm not very articulate when it comes to explaining things in writing I found this online..
During second trimester ultrasounds, done in some places on a routine basis, will show that there are many more previas diagnosed at this stage. Typically at 16 weeks the placenta takes up 25-50% of the surface area. Also the third trimester brings a growth of this lower uterine segment, that out flanks the growth of the placenta. For these reasons, while 5% of pregnancies are diagnosed with complete previa in second trimester ultrasounds will see 90% of them resolved by term and while 45% of pregnancies are diagnosed with marginal previas will see 95% resolved at term.
and as for the management of the pregnancy you OB will be able to answer all your questions but I found this:
Placenta previa, once diagnosed, will usually mean bed rest for the mother, frequently in the hospital. Depending on the gestational age steroid shots may be given to help mature the baby's lungs. If the bleeding cannot be controlled immediate cesarean delivery is usually done, regardless of the length of the pregnancy.
Do you live very far from the hospital? They may put you on bed rest once you reach 24 weeks. I hope everything goes well for you

Thanks for that info Mrs B! So it does seem like if it is going to resolve, then it may not happen til the 3rd trimester - and I didn't make it that far last time! We are in Cambridge so about 25 mins from the hospital. Bedrest doesn't sound very appealing Are you still working in NICU at Waikato?

Yep I'm back at work two days a week. Hopefully don't see you there! Were they able to give you a reason for Ashleigh's early arrival? Just because you have had one prem doesn't automatically mean you will have another
Yep I imagine bedrest sucks the big kumera but if that's what it takes...

Yip absolutely. It would be Ashleigh that I would be more concerned about, not myself! And I also hope to not see you there haha!!!

There was no reason for Ashleigh's early arrival. I've been told I just have a higher chance than the average person of having another prem. And I think I've started having braxton hicks in the last couple of days which is also freaking me out!

I had placenta previa with ds2, it didn't move but once I was diagnosed I was watched really closely unless there is any other complecations they will bring you in to hospital around 34 weeks just to make sure that if you go into labour you are in the right place.
I had a lot of bleeds so ended up in hospital before that and they decided to take the baby at 34weeks but it was because they suspected it was an acreta as well.
I wasn't able to lift ds1 for most of the pregnancy and my mum had to come over from Ireland for 3 months to help out. I have a cousin how has had it on 2 of her pregnancy one was a section the other the placenta moved and she had a vbac.
Hope i haven't freaked you out to much

I had it at the 20 week scan but had another one at 32 weeks and it had moved by then.

I didn't have a full previa - but was still too close to cervix, needed to be 2cm away and was only 1cm. At 32 weeks it hadn't moved but by 36 weeks it had.

me too.
I did a lot of research into it between 32 and 36 weeks - basically it's really common in first trimester but only a few persist beyond the second trimester. Hopefully yours will be resolved by 20 weeks and you can relax. If not, given your history, I would push for an earlier follow-up scan - perhaps 25 weeks instead of the routine 30 weeks.
If it helps, my mum was hospitalised with bleeding from plac. previa for a week around 14 weeks with my bro but it resolved by 20 weeks.
Fingers crossed for you!
ETA. sorry, just read properly that you are getting monthly scans anyway, so hopefully you get to watch it move soon.

Yeah I was gonna say what TRex said...maybe the reason that lots of them are resolved in third trimester is more to do with when the follow up scans are? ie maybe if they did a follow up scan earlier they would show to resolve earlier...whereas coz you are getting yours monthly as TRex said, you might get to see it move and hopefully earlier :-)
