What I did with both Amy and Sophie was to (sort of) follow the Gina Ford Contented Little Baby routine. This suggest starting your day at 7am regardless of what has happened the day before and during the night. This also means treating anthing bafore 6.30am as night time.
So at 4 months Sophie was on the following routine:
7am - up, nappy change, bottle (200mls)
8/8.30am - dressed and back to bed (luck thing!)
10 - up and bottle about 10.30
12/12.30 - back to bed
2.30/3 - up and bottle
5.30 - half a bottle (she could never last until bedtime
6/6.30 - PJs rest of bottle, bed
10pm - dream feed
We dropped the dream feed at about 5 months (I think

can't remember!!) and the routine stayed pretty much the same even now, with some changes for solids (10.30 bottle is gone and lunch is at 12ish then bed at 1pm)
Sorry this is so long, some people hate Gina Ford but she suits me and both my girls - they are real routine babies
eta: a friend of mine moved her second son on to this routine when he was 4 months(he is now 6 months) and he dropped the middle of the night feed within about a week. Her previous routine was similar to yours. Good luck
