That's exactly what my Mum did when Paul was a baby, we were all at school/preschool and he was the only one home with the nanny while she worked so she used to pretend to go to work then the nanny took Paul out for a walk and Mum would sneak back in to her bedroom (her computer was in there and she was a typist) and close the door and he never knew she was there.
I don't know if I'd want to do that tho, I kind of like that I can pop out and see the girls whenever I want to, or give them cuddles when they first wake up etc. It's one of the bonuses of a nanny vs. daycare.
